It would take a long crank or multiple cranks to fire up. It eventually just wouldn't fire at all. Turned out to be the fuel pump. This was in 2001 on a '99 5.9L. My mechanic said it must have been a bad design from the factory since it took two weeks to obtain the OEM replacement. Lots of demand in a short period for that specific part was unusual. Also, remember that the pump needs fuel to stay lubed. If you continually run your tank down, the pump will overheat and prematurely fail.
