Lug nut Problem!

I've heard about people having lugnuts that strip, but I ran into something strange. I recently had my tires rotated and noticed a few days later that I was missing a couple chrome lugnuts. After looking a little closer, I realized that I was only missing a chrome cap on the lug nut.

WHAT THE IS THAT? Each lug nut has a chrome coated cap over the actual lug nut. Somehow during the rotation, a couple came loose and now I am missing caps with another couple that are loose.

Any one else have the came problem?
