Those are V-Tech "originals" and I got them from Corporate Member AAC.

As I remember, they cost me $25 but I also paid $45 to have a local dent repair place color match paint them for a total of $75. You can get them already "painted to match" from Caribbean Custom (www.caribbeancustom.com/) for only $55 with free shipping inside the US.

They mount using 3M's body moulding tape (comes preinstalled on the rear edges of the covers) just be sure not to press 'em down tight until you get the alignment right!
