The Waiver is available on-line in the following formats:

Adobe PDF format - Print it and fill it out. (requires Acrobat Reader)
MS Word format - Download it, open it, print it and then fill it out. (requires MS Word or MS Word Viewer)

After filling it out, please submit it to the event organizers. Thank you!
